A low-grade periprosthetic joint infection (PJI) may be difficult to diagnose, and nuclear imaging could help in the diagnosis. However, its diagnostic value is unclear. We retrospectively evaluated this diagnostic value. We conclude that in patients presenting with nonspecific symptoms and a low a priori chance of PJI based on clinical evaluation, nuclear imaging is of no clear added value in diagnosing a PJI.

Despite potential pharmacokinetic advantages, the local application of antibiotics in the treatment of periprosthetic joint infections has so far been associated with bad outcomes. This study shows a major advantage for the local application of antibiotics with calcium sulfate as the carrier material in the implant-retaining treatment of a periprosthetic joint infection of the hip.

The yearly incidence of native vertebral osteomyelitis (NVO) is increasing. In 2013 the prevalence of NVO in the USA was 5.4 cases per 100 000. Delays in the diagnosis of NVO remain common, in part due to the insidious nature of the infection and its related symptoms. The mean time between the onset of symptoms to diagnosis of NVO is 45.5 d. We herein review available and novel diagnostic modalities at the disposal of healthcare providers to reach an accurate and timely diagnosis of NVO.

This case illustrates the clinical picture of a worn-out elbow prosthesis resulting in severe metallosis and a subsequent periprosthetic joint infection.

This study consists of 26 patients with infection of the pubic symphysis. The authors believe that by drawing awareness to this condition patients may be diagnosed and undergo surgery faster; 21 of the patients had pelvic surgery before onset of pubic infection. The diagnostic delay was between 1 and 12 months. All patients underwent surgical debridement and received a minimum of 6 weeks antibiotic treatment; 23 patients had postoperative pain relief and 19 patients were ambulant without aids.

Early infection after primary total hip arthroplasty can be effectively treated with one-stage cementless exchange revision. In our retrospective case series we have 100% infection control. If other similar orthopedic centers are able to reproduce our results, the potential benefit for the patients and treating institutions is of considerable importance worldwide. We did this research to document our results and make the protocol available for the orthopedic infection surgery community.

Areas in orthopedics, such as spinal metastasis and sarcomas, have shown benefits of concentrated care in specialized centers. Currently in the US, no such centers exist for treatment of periprosthetic joint infection (PJI). We established a PJI center and review our first-year experience. Additionally, we note the complexity and volume of referrals, in addition to the abundant opportunities for research that referral centers provide. PJI centers are feasible and can improve PJI care.
